For new patients living in IOWA or MINNESOTA who have not yet been evaluated or certified for medical cannabis use by The Herb For Health Clinic. The certification lasts 3 years from the date the application is approved.
Patients living in IOWA or MINNESOTA who are enrolled in the Medical Cannabis Program must have their qualifying condition(s) recertified and complete an application to re-enroll every three years.
Registered patients are allowed to have another participating health care practitioner certify their condition.
*** Recertification appointments can be requested up to 90 days before the patient's enrollment in the program expires.
